Frequently Asked Questions

Discounts may vary due to optical locations in your area. Call your selected optical location directly for specific discounts.

You and your family have the opportunity to utilize the EyeBenefits Vision Discount Program as many times as you feel necessary. There are no limits to the number of eye exams, eyewear and contact lenses that you can purchase throughout your EyeBenefits Membership.

Visit the EyeBenefits website homepage and click on the “Sign Up Online” tab. Enter your group code to receive your group rate. If you don’t know you group code call your organization’s benefit department. Proceed by clicking “Next” and continue through the enrollment process. To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card.

Visit the EyeBenefits website homepage and click on the “Sign Up Online” tab.Leave the group code field empty and click “Next”. To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card.

Your family members do not need their own Membership E-Cards to receive the discounts.

All cancellation refunds will be given no later than 30 days after the Member receives their EyeBenefits Membership Card. For questions, concerns or complaints please call Customer Care at 800-621-7900. All inquires will be acknowledged in writing within 5 business days and the results of the investigation will be provided no later than 30 days after the complaint is received.

If you enrolled online: Visit the EyeBenefits website homepage and click on the “My Account” tab. You will then be prompted to enter in your Username and Password. If you do not know your Username or Password call EyeBenefits Customer  Care at 800-621-7900.

If you received your Membership E-Card through your company’s Human Resource Department: Please contact your Human Resource Director for further assistance.

Maybe.  EyeBenefits may be used with your current insurance plan if the Provider you have selected allows it.  Please check with your Provider when you call for an appointment.

Visit the EyeBenefits website homepage and click on the “Optical Location” tab. Enter both the city and state or the zip code, using only the first three digits of the zip code will broaden the search area. A list of all the optical locations will be displayed.

All optical locations listed on the EyeBenefits website have been contracted and credentialed with EyeBenefits. If an optical location is uncertain if EyeBenefits is in their system, please have them call EyeBenefits Provider Relations directly at 480-659-2850.

Visit the EyeBenefits website homepage and click on the “Become A Provider” tab and complete the information fields.

EyeBenefits Members will receive the better of their EyeBenefits vision discount or the sales price, whichever is the better discount, however it is the optical locations decision to combine the discounts.
